EARLY VOTING

Beginning August 3, 2016, individuals who are eligible to
vote in this election and who wish to vote an early ballot
in person may appear at any of the County Recorder
Offices located at:
510 S. 3rd Ave., Phoenix,
111 S. 3rd Ave., Phoenix, or
222 E. Javelina Ave., Mesa.

Friday preceding the Primary Election. Written
requests should include:
1) Name and residence address;
2) Birth date;
3) Election for which the ballot is requested;
4) Address where the ballot is to be mailed if different
than residence address;
5) Signature of requestor.

In order to be valid and counted, the mailed ballot and
signed Affidavit must be received by the Office of the
County Recorder or other officer in charge of elections,
or deposited at any polling place in the county no later
than 7:00 p.m. on Election Day, August 30, 2016.

PERMANENT EARLY VOTING LIST VOTERS
Voters who have signed up to be on the Permanent Early
Voting List and who are eligible to vote in this election
will receive their ballot in the mail automatically. The
ballots will be mailed out to reach the voter on or about
the first day for early voting, August 3, 2016. If you do
not wish to receive an early ballot by mail, you must
contact Maricopa County Elections and indicate: 1) you
do not wish to receive an early ballot for this election,
2) you have changes to your voter information on file or
3) wish to be removed completely from the Permanent
Early Voting List.
Los votantes que se han registrado en la Lista de
Votación Temprana Permanente y que son elegibles para
votar en esta elección, recibirán su boleta en el correo

IDENTIFICATION AT THE POLLS
LIST #2: Non-photo identification (name and address
only) – TWO REQUIRED
Acceptable forms of identification without a photograph
that bears the name and address of the elector:
• Utility bill of the elector that is dated within 90 days of
the date of the election. A utility bill may be for electric,
gas, water, solid waste, sewer, telephone, cellular
phone, or cable television
• Bank or credit union statement that is dated within
90 days of the election
• Valid Arizona vehicle registration or vehicle insurance
card
• Indian census card, Tribal enrollment card or other
form of tribal identification
• Property tax statement
• Recorder’s certificate or Voter Registration card
• U.S. Federal, State or Local Government issued
identification
• Any “Official Election Material” mailing bearing the
voter’s name and address
Every qualified elector at a polling place is required to
show proof of identity before receiving a ballot. The
elector shall announce his/her name and place of
residence to the election official and present one form of
identification from List #1 that bears the name, address,
and photograph of the elector OR two different forms of
identification from List #2 that bear the name and address
of the elector OR two different forms of identification
from List #3.
An elector who does not provide one form of identification
from List #1 that bears the name, address, and
photograph of the elector OR two different forms of
identification from List #2 or List #3 that bear the name
and address of the elector shall not be issued a regular
ballot, but shall receive a provisional ballot and will
have five (5) days after a Federal General election
and three (3) days after any other election to provide
sufficient ID to the County Recorder in order for their
provisional ballot to count.
ID MUST reasonably match the precinct register.
Identification is “valid” unless it can be determined on its
face that it has expired.
LIST #3: Mix and Match from List #1 and List #2 –
TWO REQUIRED
• Any Valid Picture identification from List #1 in which the
address does not reasonably match the precinct
register or that does not contain an address
accompanied by a non-photo identification from
List #2 in which the address does reasonably match
the precinct register
• U.S. Passport without address and one item from
List #2
• U.S. Military identification without address and one
item from List #2
LIST #1: Photo identification with name and address
– ONE REQUIRED
Acceptable forms of identification with photograph, name,
and address of the elector:
• Valid Arizona driver license
• Valid Arizona non-operating identification license
• Tribal enrollment card or other form of tribal identification
• U.S. Federal, State or Local Government
issued identification

OFFICIAL TITLE: Ratification of the City of Glendale General Plan, Envision Glendale 2040.
DESCRIPTIVE TITLE: Proposed ratification of the City of Glendale General Plan, pursuant to A.R.S. §
9-461, et. seq., as set forth in Resolution No. 5095 adopting the Glendale General Plan, approved by the
Council of the City of Glendale on April 26, 2016. The General Plan establishes planning goals, objectives
and land use policies in the City. It does not raise any taxes.
QUESTION: Shall the City of Glendale General Plan as set forth by Resolution No. 5095, adopted by the
Glendale City Council on April 26, 2016 be approved?
A “YES” vote will have the effect of approving Proposition 497 and ratifying and adopting the City of
Glendale General Plan as approved by the Glendale City Council.
A “NO” vote will have the effect of rejecting Proposition 497 and rejecting the City of Glendale General
Plan as approved by the Glendale City Council and retaining the current City of Glendale General Plan
(2002).

conformidad con A.R.S. § 9-461, y ss., tal como se expone en la Resolución Núm. 5095 adoptando el
Plan General de Glendale, aprobado por el Concejo de la Ciudad de Glendale el 26 de abril de 2016. El
Plan General establece metas de planificación, objetivos y normas de uso de terrenos en la Ciudad. No
aumenta impuestos.
PREGUNTA: ¿Debe aprobarse el Plan General de la Ciudad de Glendale tal como se expone en la

WHEREAS, the City intends to establish policies for

review period and the written comments and public
hearing testimony received pursuant to such required
review have been duly considered; and

was presented at public hearings conducted by the
Glendale Planning Commission at two different locations
in the City of Glendale on April 23, 2016, and April 25,
2016, respectively, and forwarded to the Glendale City
Council for consideration of Envision Glendale 2040
together with recommended text and land use map
amendments derived from the sixty-day review period
and public testimony, and

directed such additional amendments and refinements
to the General Plan as it deems necessary, if any; and

SECTION 1. That the City of Glendale adopts the
City of Glendale General Plan, Envision Glendale
2040, in its entirety as generally described:

The City Council formed the General Plan Steering
Committee to ensure an open process for community

activities enumerated to fulfill requirements of Arizona
Revised Statutes 9-461.06 (B)(1) for citizen input
were: regular meetings with the Steering Committee,
area meetings, community workshops, briefings to
Boards and Commissions and civic events as well as
presentations on KGLN 11 and at City Council district
meetings. Planning coordination with other jurisdictions
and agencies, incorporation of public planning studies
and community outreach efforts were also utilized to
achieve a General Plan with a strong foundation of
grassroots values and opinions.

The Proposition, if adopted, would approve Envision
Glendale 2040, the Glendale General Plan. On April
26, 2016, the Glendale City Council approved Envision
Glendale 2040. State law requires that, once approved
by the City Council, Envision Glendale 2040 must
be sent to the voters for ratification of the Council
approval. Envision Glendale 2040 does the following
in the Glendale Municipal Planning Area:

with particular attention to the General Plan Adoption;
relationship of the General Plan to other City plans,
along with future Major and Minor General Plan
Amendments.

The City Council formed the General Plan Steering
Committee to ensure an open process for community
residents and stakeholders to become actively involved
in the preparation of the General Plan. Among the
activities enumerated to fulfill requirements of Arizona
Revised Statutes § 9-461.06 (C)(1) for citizen input
were the following: regular meetings with the Steering
Committee, area meetings, community workshops,
briefings to Boards and Commissions and civic events
as well as presentations on KGLN 11 and at City
Council district meetings. Communication methods,
planning coordination with other jurisdictions and
agencies, incorporation of public planning studies
and community outreach efforts were also utilized to
achieve a General Plan with a strong foundation of
grassroots values and opinions.

especial atención a la Adopción del Plan General; la
relación del Plan General con otros planes de la Ciudad,
conjuntamente con futuras Enmiendas Mayores y
Menores al Plan General.

The Glendale Chamber of Commerce, representing
over 1,000 businesses in the Glendale and West Valley, urges voter support for “Envision Glendale 2040”
General Plan update. The General Plan is the official
policy document of the Glendale City Council to guide
the private and public development of the city in a
manner that reaps the maximum social, economic, and
environmental benefits for the citizens of Glendale.
Members of the business community have participated
in the General Plan Public Focus Group process, and
concur with the plan’s recommendations for the future
growth of our city. Envision Glendale truly is a community plan that is business friendly and supports our
neighborhoods.
La Cámara de Comercio de Glendale, representante de
más de 1.000 negocios en Glendale y West Valley, insta
a los votantes a que respalden la actualización al Plan
General Visión de “Glendale 2040”. El Plan General
es el documento de normativas oficial del Concejo de
la Ciudad de Glendale que guía el desarrollo privado y
