All new patients are required to provide the following documents for their first visit to our center.

A Referral or Authorization, if required by insurance from your primary doctor
Insurance Card
Social Security Card
Proof of Address ( For example: Con Edison)
Picture ID
Immunization cards( Pediatrics Only)
The name, address and phone number of primary doctor.
The name, address and phone number of pharmacy.
If the patient is under 18 years of age, a parent or legal guardian must accompany the patient.
o Legal guardian must bring proof of guardianship, picture ID and proof of address.
o If the minor is accompanied by someone other than a legal guardian picture ID with a
notarized letter signed by the legal guardian is required.
If the patient is under 18 years of age and the patient has been declared an emancipated
minor by US court of law, documentation must be provided.
Copayment amount designated by insurance carrier is expected at the time of service. Cash,
check or credit card accepted.
Self Pay patients without insurance must bring tax returns from the previous year to
determine sliding scale fee. Payment is expected at the time of service.

Si el paciente ene menor de 18 anos su pariente o tutor legal tiene que acompanar el
paciente.
o Tutor legal debe traer prueba legal del guardian o tutor y tambien prueba de
identificacion y direccion
o Si el paciente viene acompanado por alguien que sea el pariente legal, el
acompanante de traer una Carta notarizoda, autorizando al acompanante
Si el paciente es menor de 18 anos con poder de tutor, debe de venir con el paciente o si es
una madre emancipada ( madre menor con hijo) declarada por ley en los Estados Unidos
Debe de traer prueba si tiene custodia legal del paciente
